The original purpose of the ISWT was to develop a standardised, externally paced, incremental field walking test to assess the functional capacity in patients with chronic heart and lung disease. The ISWT is a valid symptom limited maximal test of functional capacity that relates strongly to VO2max during cardio-pulmonary exercise testing on a treadmill.
